Are you or someone you know living with diabetes? Diabetes Grey Bruce provides education, support, case management and coaching to adults and children with diabetes, as well as those at risk of developing the disease.

A certified diabetes educator can help people confidently manage their diabetes so they have better blood sugar control, fewer visits to the emergency department, and shorter stays when they do have to go to the hospital.

Diabetes Grey Bruce outpatient clinics are located in hospitals in Hanover, Kincardine, Owen Sound, Meaford, Wiarton, Markdale and Southampton. Outreach clinics are offered at Chippewas of Nawash Unceded First Nation, Saugeen First Nation, Bruce Power, the South East Grey Community Health Centre and more.

When you arrive, your team, made up of registered nurses, registered dietitians, and a nurse practitioner, will learn your history, help you set health goals, and provide one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices. Staff also have specialized training on insulin pumps, childhood diabetes, gestational diabetes, and diabetes screening for those with mental illness or undergoing cancer treatment.
